About the Role

In this full-time position, you'll support clients through every phase of their housing journey using a "whatever it takes" approach. Your work spans street outreach, interim housing, and permanent supportive housing settings, with a focus on building trust and connecting people to the resources essential for long-term stability.

What You'll Do

  • Conduct comprehensive intake and individualized assessments to understand each client's needs, strengths, and goals
  • Collaborate with clients to develop Individualized Service Plans addressing housing, income, health, mental health, substance use, finances, employment, and social support
  • Administer clinical assessments (including VI-SPDAT) to guide service planning and care coordination
  • Maintain confidential, detailed case documentation in compliance with HIPAA, agency policy, and funding requirements
  • Schedule 90-day progress reviews and adjust support strategies as clients' situations change
  • Provide transportation and meet clients in community settings, as driving is a core function of this role
  • Connect clients with external resources, referrals, and services to support their personal goals

What We're Looking For

  • Two years of experience in case management, healthcare, homeless services, or supporting vulnerable populations (preferred)
  • Analytical mindset with meticulous attention to detail in documentation and data management
  • Clear, professional communication skills—both written and verbal—with diverse populations
  • Proactive problem-solver who initiates solutions and adapts flexibly to complex, changing circumstances
  • Collaborative approach to teamwork; ability to build positive working relationships with colleagues and clients
  • Strong ethical foundation and commitment to professional boundaries and client confidentiality
  • Valid driver's license, reliable transportation, and current proof of insurance and vehicle ownership
  • CPR/First Aid certification (or commitment to obtaining it); willingness to complete TB testing, background screening, and drug screening
  • Authorization to work in the United States

Our Supportive Housing Services provide intensive case management within permanent housing settings for individuals with histories of homelessness and co-occurring chronic health, mental health, or substance use conditions.   • Salary: $23.24–$28.28 per hour, based on experience and qualifications
