About the Role

As a Case Manager with PATH's Supportive Housing Services team, you'll work with clients experiencing chronic homelessness or complex barriers to stability—including health conditions, mental health challenges, and substance use histories. Using an intensive, individualized approach, you'll help clients navigate every stage of their housing journey, from street outreach through permanent housing stabilization.

What You'll Do

  • Conduct comprehensive intake interviews and assessments to understand each client's unique circumstances and goals
  • Collaborate with clients to create detailed service plans that address housing, employment, medical, mental health, and personal development priorities
  • Connect clients with community resources, social services, medical providers, and employment opportunities aligned with their plans
  • Administer program-specific assessments to identify client strengths and barriers to housing stability
  • Track client progress through 90-day reviews and ongoing touchpoints to adjust support as needed
  • Maintain thorough, accurate client documentation and case files in compliance with program standards and confidentiality regulations

  • Preferred: Two years of experience in healthcare, case management, homelessness services, or supporting vulnerable populations
  • Mandatory: Valid US driver's license, reliable personal vehicle, proof of insurance, and willingness to pass CPR/First Aid certification, tuberculosis screening, background check, and drug test
  • Must be authorized to work in the United States
