About the Role

In our Permanent Supportive Housing program, you'll serve clients navigating chronic homelessness with complex health, mental health, and co-occurring needs. You'll employ a comprehensive, person-centered approach to support individuals through every phase of their journey—from street outreach to permanent housing—breaking down barriers and fostering long-term stability.

What You'll Do

  • Perform intake assessments and work collaboratively with clients to develop personalized service plans targeting housing security, income stability, health, mental wellness, substance use support, employment, and social connection
  • Link clients to community resources and services that support their individualized goals
  • Complete validated assessments (including VI-SPDAT) to guide care planning
  • Conduct regular progress reviews with clients, identify obstacles, and adapt support strategies accordingly
  • Maintain complete, confidential documentation in compliance with HIPAA and organizational standards

Qualifications: Two years of experience in healthcare, case management, homelessness services, or direct support with vulnerable populations is preferred. All educational backgrounds and experience levels welcome.

Requirements: U.S. work authorization required. Current CPR/First Aid certification or willingness to obtain it. Must pass pre-employment TB test, background screening, and drug test. Valid driver's license with reliable personal vehicle (proof of insurance and ownership required); ability to qualify for PATH's vehicle insurance.

Compensation & Support

$23.24 to $28.28 per hour (based on experience), paid bi-weekly over 24 annual pay periods. Benefits include medical, dental, and vision coverage; paid vacation and sick leave; paid holidays; and retirement plan eligibility.
