What You'll Do Perform intake assessments and work collaboratively with clients to develop personalized service plans targeting housing security, income stability, health, mental wellness, substance use support, employment, and social connection Link clients to community resources and services that support their individualized goals Complete validated assessments (including VI-SPDAT) to guide care planning Conduct regular progress reviews with clients, identify obstacles, and adapt support strategies accordingly Maintain complete, confidential documentation in compliance with HIPAA and organizational standards What We're Looking For Core competencies: Analytical and detail-oriented approach to work and documentation Collaborative mindset that fosters strong relationships with team members, clients, and community partners Excellent verbal and written communication skills, capable of explaining complex information clearly and compassionately Proactive problem-solver who takes initiative and finds creative solutions Qualifications: Two years of experience in healthcare, case management, homelessness services, or direct support with vulnerable populations is preferred. All educational backgrounds and experience levels welcome. Requirements: U.S. work authorization required. Current CPR/First Aid certification or willingness to obtain it. Must pass pre-employment TB test, background screening, and drug test. Compensation & Support $23.24 to $28.28 per hour (based on experience), paid bi-weekly over 24 annual pay periods.
