Join PATH (People Assisting The Homeless) as a CalAIM Case Manager in San Diego County. In this full-time position, you'll guide individuals experiencing homelessness toward stable housing and improved health outcomes through comprehensive case management and resource coordination.

About the Role

PATH's CalAIM program partners with managed care plans to serve Medi-Cal beneficiaries experiencing homelessness, including those with chronic homelessness and complex health or behavioral needs. As a CalAIM Case Manager, you'll employ a "whatever it takes" philosophy to help participants break free from homelessness, addressing not just housing but also healthcare, mental health, substance use recovery, financial stability, and vocational goals.

What You'll Do

  • Perform detailed intake assessments and work with each participant to develop individualized service plans addressing housing, income, medical, mental health, substance use, financial, career, and social needs
  • Connect participants with community resources, services, and support networks aligned with their service plan goals
  • Administer assessment tools to identify service priorities and guide appropriate interventions
  • Conduct progress reviews with participants to evaluate goal achievement and adjust plans accordingly
  • Maintain comprehensive, timely documentation and complete case file requirements per program and PATH policies

What We're Looking For

  • Strong organizational skills and attention to accuracy in all work
  • Ability to work effectively as part of a team and build positive relationships with colleagues and participants
  • Clear communicator, both verbally and in writing
  • Proactive, creative problem-solver who takes initiative
  • Sound judgment, ethical practice, and professional boundaries
  • Preferred: Two years of experience in healthcare, case management, homelessness services, or work with vulnerable populations

Requirements

  • Valid driver's license with reliable transportation—driving is essential to this position
  • CPR/First Aid certification or willingness to obtain it
  • Pass background screening, TB test, and drug screening as a condition of hire
  • Authorization to work in the United States
