What This Role Involves

As a Social Worker with Spectrum Health Care, you'll provide compassionate, professional support to individuals and families navigating health challenges and life transitions. You'll assess client needs, develop care plans, and work collaboratively with our healthcare team to connect people with the resources and services they need. Your expertise will help bridge the gap between medical care and social support, ensuring clients receive holistic, person-centred assistance.

Day-to-Day Responsibilities

  • Conduct comprehensive assessments of clients' social, emotional, and environmental circumstances
  • Develop and implement individualized care and support plans
  • Provide counselling, crisis intervention, and emotional support to clients and families
  • Coordinate and refer clients to community resources, social services, and support programs
  • Maintain detailed, accurate case notes and clinical documentation
  • Advocate for clients' rights and needs within the healthcare system and broader community
  • Collaborate with physicians, nurses, and other healthcare professionals
  • Participate in team meetings and contribute to care planning discussions

Qualifications

  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree from an accredited program
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), or eligibility to register
  • Strong communication and interpersonal skills with the ability to build rapport with diverse populations
  • Experience with case management, assessment, and care coordination in a health care setting (preferred)
  • Ability to work independently and as part of a multidisciplinary team
  • Proficiency with electronic health records and case management systems
  • Commitment to cultural competence and trauma-informed practice
