Position Summary

What You'll Do
- Conduct comprehensive psychosocial assessments to understand client needs, circumstances, and goals

- Facilitate connections to appropriate health, social, and community services and benefits
- Maintain accurate, confidential client records and documentation
- Collaborate with interdisciplinary teams to coordinate integrated client services
- Advocate for client rights and ensure equitable access to supports and programs
Requirements
- Bachelor's degree in Social Work or equivalent accredited qualification
- Current or eligible membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Demonstrated experience in social work practice within health care or community settings
- Strong communication, active listening, and interpersonal skills
- Ability to work collaboratively within multidisciplinary health care teams
- Commitment to client confidentiality, professional ethics, and cultural sensitivity
