LPN/RN Pediatric Home Care – Weekend Days

Phoenix Home Care and Hospice is seeking a compassionate Licensed Practical Nurse or Registered Nurse to join our pediatric home care team in Saint Paul, Missouri, providing weekend day shifts to children and their families. This full-time position offers the opportunity to deliver direct nursing care in the comfort of patients' homes, building meaningful relationships while making a real difference in young lives.

About the Role

As an LPN/RN Pediatric Home Care professional, you'll provide skilled nursing care to pediatric patients in their home environments on weekend days. You'll be an essential partner in our mission to support children with complex medical needs, ensuring they receive compassionate, high-quality care while enabling families to thrive outside hospital settings.

Day-to-Day Responsibilities

• Deliver direct nursing care to pediatric patients, including medication administration and wound care
• Monitor and record vital signs, clinical observations, and patient progress
• Assess patient conditions and report changes to the clinical team
• Assist patients with activities of daily living and personal care needs
• Educate patients and families on health management and treatment plans
• Maintain detailed, timely documentation and clinical records
• Collaborate with physicians, therapists, and home care coordinators to ensure coordinated care
• Follow infection control and safety protocols in home-based settings

What We're Looking For

• Current, valid LPN or RN license in Missouri (or eligible for reciprocity)
• Experience in pediatric nursing or willingness to work with children
• Current CPR/BLS certification (pediatric certification preferred)
• Strong communication skills and ability to work collaboratively with families
• Reliable transportation and valid driver's license
• Compassion, patience, and genuine care for pediatric patients
• Ability to adapt to diverse home environments and patient needs
• Weekend day availability and scheduling flexibility
