Phoenix Home Care and Hospice is hiring a full-time Licensed Practical Nurse (LPN) or Registered Nurse (RN) specializing in Private Duty Nursing for children and young adults in Centralia, Missouri. This is an essential role providing compassionate, skilled nursing care to pediatric and young adult clients in home settings.

Day-to-Day Responsibilities

• Perform direct patient care including medication administration, wound care, vital sign monitoring, and specialized medical procedures
• Assess patient health status and maintain accurate, detailed clinical documentation
• Educate families and caregivers on care techniques, medication management, and recognizing changes in patient condition
• Communicate regularly with physicians and care team members to coordinate treatment plans
• Monitor medical equipment and supplies, ensuring proper function and ordering as needed
• Support patients' physical, emotional, and developmental needs within a home care setting

Who We're Looking For

• Current LPN or RN licensure in Missouri
• Experience or strong interest in pediatric and young adult care
• Excellent clinical assessment and critical thinking skills
• Compassionate communication with families and patients
• Ability to work independently while maintaining professional collaboration
• Strong documentation and time management abilities
