Care Manager

Entyre Inc is hiring a Care Manager to support family caregivers across Phoenix, Arizona and beyond in this full-time remote position. Oversee in-home assessments, guide personalized care planning, and ensure caregivers have the training and oversight needed to deliver quality care to their loved ones.

Position Summary

We're looking for a Care Manager to serve as the clinical and operational foundation of our in-home caregiving program. You'll conduct home assessments, design and monitor individual care plans, train caregivers on evidence-based practices, and track ongoing care quality. This role bridges clinical oversight with direct family support—you'll ensure caregivers feel equipped and confident while maintaining full compliance with state regulations and care standards. Working remotely from the Phoenix area on a full-time basis, you'll directly improve outcomes for families navigating in-home care.

What You'll Do

• Perform in-home assessments to evaluate client health needs and caregiver competency
• Develop and update personalized care plans that reflect each family's specific health goals and preferences
• Train caregivers on proper care techniques, safety protocols, and approaches tailored to individual client needs
• Conduct regular care monitoring through home visits, progress reviews, and direct feedback with families
• Document and report client health status, care progress, and any clinical changes to relevant stakeholders
• Ensure all care delivery meets state requirements, licensing standards, and organizational quality benchmarks
• Serve as a trusted advisor to families, addressing their questions and troubleshooting care challenges

Skills & Qualifications

• Previous experience in care management, clinical assessment, case management, or related healthcare roles
• Understanding of in-home care delivery, adult health standards, and regulatory compliance requirements
• Clear written and verbal communication with ability to document assessments and brief team members
• Highly organized with strong attention to detail when managing multiple client caseloads
• Self-directed and resourceful; comfortable working independently within organizational guidelines
• Genuine empathy for families facing caregiving challenges and commitment to supporting their wellbeing
