What This Role Involves

As a Care Management Support Coordinator III, you'll serve as a vital connecting point between members, healthcare providers, and our care management team. Your Responsibilities

• Conduct regular member outreach via phone to provide guidance on care plans, available health plan and community resources, and service scheduling
• Answer and resolve inquiries from members and healthcare providers regarding care management procedures, protocols, and available support services
• Connect members with relevant health plan resources and community services to ensure comprehensive, high-quality care coordination
• Support member onboarding by preparing and distributing welcome communications and educational materials
• Schedule services and coordinate logistics to facilitate successful member and provider relationships
• Maintain accurate, compliant documentation of member interactions and records in accordance with regulatory requirements
• Escalate complex requests and concerns to management for appropriate resolution
• Assist with training and onboarding of new team members
• Perform additional duties as assigned by the care management team

Skills & Qualifications

• High School diploma or GED required
• 2 to 4 years of direct experience in care coordination, member support, administrative healthcare, or a related field
• Strong verbal communication skills and ability to interact professionally with members, providers, and internal teams
• Proficiency with phone-based customer service and documentation systems
• Organizational skills and ability to manage multiple priorities while maintaining accuracy
• Knowledge of health plan operations and community resources
• Ability to work collaboratively within a team-oriented environment
