Physical Therapist Assistant

Phoenix Home Care and Hospice is seeking a Full-Time Physical Therapist Assistant to join our team in Lake Ozark, Missouri. This is an opportunity to deliver compassionate rehabilitation services directly to patients in their homes while supporting their recovery and quality of life.

About the Role

As a Physical Therapist Assistant with Phoenix Home Care and Hospice, you'll work within our home care team to provide therapeutic interventions under the supervision of a licensed Physical Therapist. You'll visit patients at their residences throughout the Lake Ozark area, implementing treatment plans tailored to each individual's medical condition, mobility goals, and home environment. Your role bridges clinical expertise with genuine, patient-centered care—making a meaningful difference in people's recovery journeys.

Responsibilities

• Implement physical therapy treatment plans as directed by the supervising Physical Therapist
• Perform therapeutic exercises, manual therapy, and functional mobility training with patients in their homes
• Monitor and document patient progress, vitals, and response to treatment
• Educate patients and caregivers on proper exercise techniques, safety, and fall prevention strategies
• Report findings and complications to the supervising Physical Therapist
• Maintain accurate, timely clinical documentation in compliance with regulations
• Communicate with the interdisciplinary care team to support coordinated patient care

Qualifications

• Graduate of an accredited Physical Therapist Assistant program
• Current, valid Physical Therapist Assistant license in Missouri
• Ability to work in home-based settings and adapt to diverse patient environments
• Strong communication and interpersonal skills
• Reliable transportation and ability to travel between patient homes in the Lake Ozark area
• Commitment to patient privacy, dignity, and compassionate care
