About This Opportunity

We are representing a confidential managed care organization in their search for a Care Manager, Behavioral Health. This remote role serves members across Illinois through a combination of face-to-face community visits and telephonic support. Our client provides integrated behavioral health care management to Medicaid populations, and this position serves as the primary point of contact for members navigating complex mental health and substance use needs.

This is a field-based care management role requiring regular travel within your assigned region to conduct in-home and community-based assessments. You'll work with autonomy while collaborating with interdisciplinary care teams, providers, and community organizations to deliver person-centered, culturally competent care coordination.

Responsibilities

  • Conduct face-to-face and telephonic assessments with Medicaid members experiencing behavioral health conditions, including serious mental illness (SMI), serious emotional disturbance (SED), and substance use disorders (SUD)
  • Complete comprehensive risk assessments and coordinate behavioral health and medical services to ensure appropriate provider engagement and treatment plan adherence
  • Serve as the primary care coordinator, facilitating interdisciplinary care team meetings and ensuring seamless communication among providers, service coordinators, and care management extenders
  • Engage members in their own communities through biannual and quarterly face-to-face visits, building trust and supporting proactive intervention
  • Address health-related social needs and improve member health literacy to positively impact healthcare outcomes and overall well-being
  • Support care transitions, crisis intervention, and peer support through evidence-based behavioral health services
  • Collaborate with internal departments, providers, and community-based organizations to link members to appropriate services
  • Ensure compliance with state and federal regulatory requirements, including IDHS, CMS, and NCQA standards

What We're Looking For

Required Qualifications:

  • Active Illinois license as LCSW, LMFT, or LCPC (provisional licenses not accepted)
  • Must reside in Illinois or a bordering state
  • Minimum 2 years of post-degree clinical experience in a behavioral health setting
  • Case management experience working with complex SMI, SUD, and SED populations
  • Ability to travel regularly to member homes and community facilities for face-to-face assessments (up to 25% travel)
  • Valid driver's license and personal vehicle liability insurance with minimum 100,000/300,000/100,000 limits
  • Willingness to participate in tuberculosis screening as this is a patient-facing role
  • Home internet service meeting minimum 25 Mbps download/10 Mbps upload speeds
  • Dedicated workspace to protect member PHI and HIPAA information

Preferred Qualifications:

  • Case Management Certification (CCM)
  • 3+ years of care coordination experience, including in-home assessments
  • Experience working with Medicaid populations
  • Field case management experience
  • Knowledge of community health, social service agencies, and community resources
  • Previous managed care experience
  • Bilingual capabilities (language proficiency testing may be required)

Pay

This position offers competitive compensation with the employer, and this role is also eligible for performance-based bonus incentives. Mileage reimbursement is provided for work-related travel between locations. This is a full-time position (40 hours per week) classified as remote with regular regional travel requirements.
