Fira-health is seeking a Registered Nurse (RN) - Case Manager to deliver skilled clinical care in Oakland County, managing complex patient cases in home settings as a full-time team member.
What This Role Involves
As a case-managing RN, you'll combine direct patient assessment, evidence-based clinical decision-making, and collaborative care coordination. You'll be the primary clinical lead for your patients, ensuring they receive appropriate skilled nursing interventions, achieve positive health outcomes, and experience excellent care throughout their home health journey.
Responsibilities
- Conduct comprehensive in-home nursing assessments and maintain accurate, compliant clinical documentation including OASIS assessments, start-of-care (SOC), recertification (recert), and resumption-of-care (ROC) records
- Deliver high-acuity patient care with specialized focus on wound management and evidence-based clinical interventions
- Develop and refine individualized plans of care in partnership with attending physicians and the broader care team
- Provide clinical supervision to licensed practical nurses (LPNs) and home health aides (HHAs) assigned to your cases, ensuring quality standards and consistency of care delivery
- Build and maintain professional relationships with discharge planners, assisted living facilities, physicians, and other referral sources to establish trust and drive appropriate patient referrals
- Target a full caseload of approximately 30 productivity points per week while maintaining OASIS accuracy and clinical quality standards that withstand peer review
- Monitor patient outcomes, including hospitalization rates and patient satisfaction scores, and participate in team meetings to continuously strengthen care protocols
Skills & Qualifications
- Active, current Michigan Registered Nurse license
- Home health nursing experience with demonstrated OASIS proficiency strongly preferred
- Reliable personal transportation with valid driver's license and current auto insurance coverage (mileage reimbursement provided)
- Strong clinical judgment and ability to manage complex, high-acuity patient needs in unsupervised home environments
- Excellent communication and relationship-building skills with patients, families, and healthcare professionals
- Commitment to ownership, accuracy, and continuous process improvement

You'll have a guaranteed earning floor of approximately 21 productivity points per week during your first 90 days. Additional rewards include a $1,000 sign-on bonus after 90 days and a $1,000 referral bonus for each new team member you bring to Fira-health. Full-time benefits include medical, dental, and vision coverage plus paid time off. Your regular schedule is Monday through Friday with occasional weekend shifts as needed.
