Registered Nurse (RN) – PRN

Role Overview

This PRN nursing position offers flexible, availability-based scheduling to provide skilled nursing services directly in patients' homes throughout Oakland County. As an RN focused on excellence in home health, you'll deliver comprehensive patient assessments, specialized wound care, and robust clinical documentation that sets the standard for our growing team. You'll work with physicians to develop individualized care plans, supervise licensed practical nurses and home health aides on your cases, and serve as a trusted ambassador to the referral sources and families we serve.

What You'll Do

• Conduct thorough in-home patient assessments and produce detailed, accurate clinical documentation including OASIS forms and episode-of-care records
• Provide direct skilled nursing care with particular expertise in wound care and high-acuity patient management
• Develop and refine patient plans of care in collaboration with physicians and interdisciplinary team members
• Supervise and coordinate care delivered by LPNs and HHAs assigned to your patient panel
• Build strong relationships with discharge planners, assisted living facilities, and physicians to earn continued referrals through reliable, excellent service
• Participate in team huddles and case conferences to drive positive patient outcomes and reduce preventable hospital readmissions

Qualifications

• Active, current Michigan Registered Nurse license
• Reliable transportation with valid driver's license and current auto insurance
• Prior home health or OASIS experience strongly preferred
