Physical Therapy Assistant (PTA)

Fira-health is hiring a full-time Physical Therapy Assistant (PTA) to provide skilled home-based rehabilitation services to patients throughout Oakland County. In this field-based role, you'll work directly in patients' homes, delivering evidence-based therapy while building Fira-health's reputation as a trusted, clinically excellent partner in the home health market.

What This Role Involves

You'll deliver skilled rehabilitation visits to patients in their home settings under the supervision and plan of care established by a licensed physical therapist. Your work centers on functional outcomes—improving mobility, strength, balance, independence, and safety in the environment where your patients actually live. You'll progress patients systematically, document their advancement, and communicate clinical changes with your supervising PT and the broader care team. A significant part of the role is patient and caregiver education: teaching sustainable home exercise programs and home safety strategies that patients can actually implement without ongoing professional support.

Key Responsibilities

• Deliver rehabilitation treatments aligned with established therapy plans, focusing on gait training, balance and fall prevention, safe transfers, and strengthening exercises.
• Teach patients and caregivers home exercise and management routines customized to their living situation and support network.
• Track patient progress using functional markers and objective measures; report changes in status to the supervising PT promptly.
• Assess home environments for safety hazards and recommend assistive devices, equipment, and environmental adjustments.
• Coordinate with physicians, case managers, and other care team members to ensure aligned, responsive patient care.
• Maintain complete, accurate documentation in a timely manner to support clinical continuity and billing integrity.

Who We're Looking For

• Active Michigan PTA license in good standing (required).
• Demonstrated experience working in home health settings (strongly preferred).
• Solid foundation in core PTA skills: mobility and gait training, transfers, balance work, fall prevention strategies, basic strengthening, and home safety assessment.
• Reliable personal transportation, valid driver's license, and auto insurance (this role requires travel to patient homes).
• Ability to safely transport and manage therapy equipment as needed.
• A clinician who takes personal ownership of outcomes and approaches your caseload as your responsibility.
• Comfort with clinical software and a mindset that technology should reduce administrative burden, not create it.
• Someone who builds credibility through follow-through, responsive communication, and genuine commitment to your patients and referral partners.

As an early team member, you'll help define the quality standards and operational approaches that will guide our growth and set the foundation for how we deliver care.

This is a full-time, field-based role paying $58.00 per productivity point, with an annual target of approximately 30 points per week (estimated earnings $75,000–$100,000). During your first 90 days, we guarantee a floor of approximately 24 points per week even if your caseload is still ramping. Additional compensation includes a $1,000 sign-on bonus paid after 90 days of employment and a $1,000 bonus for qualified referrals. Full-time benefits include medical, dental, and vision insurance, plus paid time off.
