Join Fira-health as a Physical Therapist serving patients in Oakland County. This is a full-time, field-based position delivering skilled in-home rehabilitation care while establishing the clinical standards that define our expanding practice in the Detroit area.

The Opportunity

Fira-health is growing its home-based physical therapy presence in the Detroit region. As an early clinician, you'll build and manage a substantial caseload while setting the operational and clinical benchmarks that guide future team members. Compensation ranges from $100,000–$130,000 annually (at $75 per productivity point, targeting ~28 points weekly), with a guaranteed minimum of ~24 points per week during your first 90 days. The package includes a $1,000 sign-on bonus (after 90 days), a $1,000 referral bonus for team growth, and comprehensive benefits including medical, dental, and vision coverage plus paid time off.

Day-to-Day Responsibilities

  • Conduct thorough in-home assessments and deliver skilled physical therapy sessions throughout your assigned service area
  • Develop comprehensive care plans addressing mobility, strength, balance and fall prevention, home safety, and functional independence
  • Set measurable therapy goals and actively track progress, adjusting your approach when patients plateau or their status changes
  • Build relationships with referral sources—discharge planners, assisted living communities, and physicians—so they consistently choose to send patients to you
  • Own your patients' complete therapy journey as their primary clinician, coordinating with the care team on adjustments and outcomes
  • Educate patients and families on practical home exercise routines they can sustain independently
  • Oversee physical therapist assistants where applicable as part of your caseload management
  • Maintain thorough, clear documentation in Homecare Homebase (HCHB) and required clinical systems
  • Travel daily across your territory and manage therapy equipment; work Monday–Friday with occasional weekend availability as needed

Skills & Qualifications

  • Valid, active Michigan PT license
  • Current driver's license and reliability with daily multi-location travel
  • Confidence working independently in varied home settings and complex patient situations
  • Ability to identify clinical changes, escalate appropriately, and document your reasoning thoroughly
  • Strong written and verbal communication
  • Competency in core home health PT: transfers, gait training, balance and fall-risk assessment, assistive device selection, therapeutic strengthening, functional mobility, and home safety education
  • Skill in creating sustainable home programs patients will actually follow
  • High ownership—you treat your caseload as your practice and team success as your responsibility
  • Home health experience strongly preferred
  • Homecare Homebase or similar home health EMR experience preferred
  • PTA supervision and mentoring experience valued
