What This Role Involves
You'll work directly in patients' homes throughout Collin County, conducting comprehensive occupational therapy evaluations and delivering skilled therapeutic interventions. Your focus is on building realistic, evidence-based treatment plans that target activities of daily living (ADLs), instrumental activities of daily living (IADLs), home safety, and functional restoration. This is field-based work requiring independent clinical judgment, strong communication with interdisciplinary care teams, and the ability to make sound decisions in real-world home environments.
What You'll Do
- Perform in-home occupational therapy evaluations to establish functional baselines and develop individualized treatment strategies.
- Design and execute comprehensive plans of care addressing ADL and IADL independence, fall prevention, and environmental safety.
- Assess, recommend, and train patients and caregivers on adaptive equipment and home modifications to support independence.
- Communicate changes in patient status to nursing and medical providers, documenting clinical reasoning with precision and clarity.
- Complete documentation efficiently using Fira-health's clinician-centered charting system, designed to reduce administrative burden and protect your clinical time.
What We're Looking For
- Active Texas occupational therapist license in good standing.
- Strong independent clinical judgment and demonstrated comfort working autonomously across diverse home settings.
- Ability to escalate changes in patient condition to nursing and medical staff with thorough clinical documentation.
- Excellent communication and documentation skills with attention to detail and clarity.
- Dependable follow-through on treatment plans, visit frequencies, and patient-centered goals.
- Valid driver's license and willingness to travel daily throughout your assigned service territory.
- Physical capability to safely handle and transport therapy equipment as part of routine care delivery.
- Core competency in home health occupational therapy practice, including functional training, home safety assessment, adaptive strategies, and caregiver education.
- Prior home health experience and exposure to high-acuity populations (neurological conditions, post-stroke recovery, fall risk management, dementia care) are valued assets.
