About the Role

The Case Manager–Care Coordinator works directly with clients experiencing homelessness across all stages of their journey—from street outreach through interim, transitional, and permanent housing. You'll employ a "whatever it takes" philosophy, combining practical problem-solving with genuine relationship-building to help clients achieve housing stability and address underlying barriers to self-sufficiency.

What You'll Do

  • Conduct comprehensive intake interviews and individualized needs assessments, identifying housing goals alongside medical, mental health, substance use, financial, vocational, and social support needs
  • Develop and implement Individualized Service Plans (ISPs) in collaboration with clients, documenting their priorities and pathways to stability
  • Complete program-specific assessments such as the VI-SPDAT to guide service prioritization
  • Facilitate connections to community resources, treatment providers, employment services, and benefit programs aligned with each client's ISP
  • Conduct 90-day progress reviews and ongoing evaluations to assess movement toward goals and adjust supports as needed
  • Maintain meticulous case file documentation and program records that meet PATH's quality and compliance standards

  • Strong analytical and organizational skills, with attention to detail in documentation and data interpretation
  • Excellent interpersonal and communication abilities—able to listen actively, convey information clearly both verbally and in writing, and build trust across diverse populations
  • Collaborative mindset and ability to work effectively within a team while independently managing client caseloads
  • Creative problem-solving approach and willingness to take initiative to remove barriers for clients
  • Sound judgment and ethical commitment to confidentiality, professional boundaries, and client dignity
  • Preferred: two years of relevant experience in case management, healthcare, homeless services, or work with vulnerable populations

Requirements and Pre-Employment Conditions

  • Valid driver's license, reliable personal transportation, and proof of vehicle insurance—driving is an essential function of this role
  • Authorization to work in the United States (visa sponsorship not available)
  • Ability to obtain CPR/First Aid certification
  • Clearance of background screening, tuberculosis screening, and drug screening
  • Must not currently receive services from PATH, nor have received services within the past two years

Compensation and Benefits

This is a full-time, non-exempt position offering an hourly rate of $23.24–$28.28 based on relevant experience, paid over 24 pay periods annually. Benefits include medical, dental, and vision coverage; vacation and sick time; paid holidays; and a retirement plan. All work is performed in California, with the primary worksite in Costa Mesa.
