RYSC SOC Family Coordinator (74636)

Prestera Health Services is seeking a dedicated RYSC SOC Family Coordinator to join our Children's Services team in Charleston, Kanawha County, West Virginia. This full-time position plays a vital role in supporting families and youth as we work toward our mission of helping people achieve their full potential and providing preferred behavioral health services to our community.

About the Role

As the RYSC SOC Family Coordinator, you'll be the bridge between families, youth, and our clinical and support services. Reporting to the RYSC Program Coordinator, you'll coordinate care, build family partnerships, and ensure youth receive the resources and support they need to thrive within our evidence-based programs.

What You'll Do

• Partner with families to identify goals, strengths, and service needs
• Coordinate between family members, youth, clinicians, and community resources
• Facilitate communication and collaboration across service teams
• Document progress and maintain accurate records of family engagement and coordination activities
• Support crisis response and service continuity when needed
• Connect families with community resources and wraparound supports
