About the Role

As a Revisit Nurse with Hartford HealthCare at Home, you'll conduct follow-up visits to patients recovering at home, monitoring their progress and ensuring continuity of care. You'll work independently and collaboratively, traveling to patients' homes across the service area to provide skilled nursing assessments, wound care, patient education, and medication support within your scope of practice as a Licensed Practical Nurse.

What You'll Do

• Perform comprehensive patient assessments, including vital signs monitoring and health status evaluation
• Assess and manage wounds, dressings, and other care needs as appropriate to LPN scope
• Administer medications and treatments according to physician orders and facility protocols
• Educate patients and caregivers on medication management, self-care, and signs requiring medical attention
• Maintain detailed, accurate clinical documentation and care records
• Communicate patient progress and concerns to the interdisciplinary care team
• Build trusting relationships with patients and families to support their recovery goals
• Manage your schedule efficiently to complete scheduled home visits across the service area

What We're Looking For

• Current Connecticut LPN (Licensed Practical Nurse) license in good standing
• Current CPR certification (American Heart Association or equivalent)
• Experience in home health, visiting nurse, or similar home-based healthcare setting
• Strong clinical assessment and patient communication skills
• Ability to work independently and use good judgment in diverse home environments
• Reliable transportation and valid driver's license
• Commitment to compassionate, patient-centered care
