About the Role
We're seeking a compassionate Licensed Practical Nurse to provide direct patient care in home and community settings across our service area. In this full-time visiting nurse position, you'll work autonomously and collaboratively to support patients recovering from illness, managing chronic conditions, or receiving end-of-life care. Based in Enfield, you'll travel to patients' homes to deliver the clinical expertise and human connection that matter most.
What You'll Do
- Perform nursing assessments and provide direct patient care, including medication administration, wound care, and vital sign monitoring
- Communicate with physicians and interdisciplinary care teams to coordinate treatment plans and ensure continuity of care
- Educate patients and families on disease management, medication compliance, and health maintenance strategies
- Maintain comprehensive, accurate clinical documentation and patient records that support quality care delivery
- Support infection control protocols and implement safe nursing practices in home environments
- Build trusting relationships with patients and families through professional, compassionate care
What We're Looking For
- Current Connecticut LPN license in good standing
- Valid driver's license and reliable personal transportation; willingness to travel throughout Hartford County
- Strong clinical assessment skills and ability to work independently with appropriate clinical supervision
- Compassion, professionalism, and excellent communication skills with patients and care team members
- Experience in home health, visiting nurse, or similar patient-centered care setting (preferred)
- Current BLS/CPR certification required
