About the Role

As a Wound Care LPN at Hartford HealthCare at Home, you'll play a critical role in patient recovery and comfort. Working from our East Hartford location (60 Hartland St), you'll travel to patient homes throughout the service area to provide direct, hands-on wound care and nursing support. This full-time position offers the opportunity to make a meaningful impact on patient outcomes while being part of a healthcare organization where every moment matters.

What You'll Do

• Assess and monitor patient wounds, documenting healing progress and changes
• Perform skilled wound dressings and care according to clinical protocols
• Collaborate with registered nurses, physicians, and care teams to optimize treatment plans
• Educate patients and families on wound care, infection prevention, and self-management
• Maintain accurate, timely clinical documentation in patient records
• Support patient comfort, hygiene, and dignity during in-home care visits
• Respond to patient concerns and report complications to supervisory staff

What We're Looking For

• Current, valid Connecticut LPN license
• Current CPR certification (BLS or equivalent)
• Experience in wound care, home health, or acute care nursing preferred
• Reliable transportation and ability to travel between patient homes
• Strong communication and interpersonal skills
• Commitment to patient-centered care and clinical excellence
• Ability to work independently and manage your schedule effectively
