This is a full-time nursing position where you'll provide direct patient care, clinical assessment, and health management support to patients recovering at home—a role where your expertise makes an immediate, meaningful difference every single day.

About the Role

As a Visiting Nurse with a primary care focus, you'll deliver skilled nursing services directly in patients' homes. You'll manage patient assessment, implement treatment plans, monitor health status, and educate patients and their families on care management and wellness. This is hands-on clinical work that requires both independence and collaboration with interdisciplinary care teams—you'll be the healthcare leader in your patients' homes.

What You'll Do

• Perform comprehensive nursing assessments and vital sign monitoring during home visits
• Administer medications, treatments, and wound care according to physician orders
• Develop and adjust patient care plans in response to changing health status
• Educate patients and caregivers on medication management, disease prevention, and self-care techniques
• Document clinical findings and progress in electronic health records accurately and timely
• Coordinate with physicians, case managers, and other specialists to optimize patient outcomes
• Respond to patient concerns and facilitate transitions in care when needed

What We're Looking For

• Current Connecticut RN licensure (or ability to obtain) required
• Clinical nursing experience, ideally in home health, primary care, or acute care settings
• Strong assessment and critical thinking skills to manage diverse patient populations
• Excellent communication and patient education abilities
• Valid driver's license and reliable transportation for visiting patients in their homes
• Ability to work independently while maintaining professional standards and safety protocols
• Compassion, flexibility, and commitment to patient-centered care
