About the Role

This full-time Revisit Nurse position offers the opportunity to build meaningful relationships with patients while providing skilled nursing care in the comfort of their homes. You'll work as part of Hartford HealthCare at Home's dedicated clinical team, supporting patients' health goals through thoughtful follow-up visits and comprehensive care management in the Rocky Hill and greater Hartford County area.

What You'll Do

• Conduct patient assessments and monitor vital signs, wound healing, and overall health status during home visits
• Provide skilled nursing care including wound care, dressing changes, and clinical procedures within LPN scope of practice
• Administer medications and provide patient and family education on medications and self-care management
• Document patient progress, observations, and clinical findings in accordance with nursing standards and regulatory requirements
• Communicate assessment findings and care updates with the interdisciplinary healthcare team
• Support patients in managing chronic conditions and recovery goals between visits
• Maintain accurate, timely clinical records and participate in quality improvement initiatives

What We're Looking For

• Current, active Connecticut LPN license (Licensed Practical Nurse)
• Experience in home health, skilled nursing, or acute care preferred
• Strong clinical assessment and documentation abilities
• Excellent communication and patient education skills
• Reliable transportation and ability to travel to multiple patient homes
• Compassionate patient-centered approach with strong attention to detail
• Ability to work independently while collaborating effectively with the care team
