About the Role
As a Revisit Nurse, you'll provide continuity of care to established patients recovering at home. This role focuses on follow-up assessments, medication monitoring, wound care, and patient education to support positive health outcomes and prevent hospital readmission. You'll work independently in home settings while collaborating with the interdisciplinary care team.
What You'll Do
- Conduct skilled nursing assessments during patient follow-up visits
- Monitor vital signs, medication compliance, and treatment progress
- Perform wound care, dressing changes, and other clinical procedures
- Document patient status and communicate findings to the clinical team
- Educate patients and caregivers on self-care and treatment protocols
- Identify patient concerns and coordinate appropriate interventions
- Maintain accurate, timely clinical records in compliance with regulatory standards
What We're Looking For
- Current Connecticut LPN license in good standing
- Experience in home health nursing, post-acute care, or related clinical setting preferred
- Strong clinical assessment and communication skills
- Ability to manage time independently and navigate multiple home visits
- Valid driver's license and reliable transportation
- Compassion for patients and commitment to quality care
- Willingness to comply with health and safety protocols
