HomeCare Aide Clinical Supervisor (LPN, LVN, Licensed Practical Nurse)

Hartford HealthCare at Home is hiring a full-time HomeCare Aide Clinical Supervisor in Hampden, Hampden County. This is your opportunity to lead a team of dedicated home care professionals, deliver clinical oversight, and ensure patients receive exceptional, compassionate care in their own homes.

About the Role

As a licensed practical nurse (LPN) or licensed vocational nurse (LVN), you'll take on a supervisory position that blends clinical expertise with team leadership. You'll oversee home care aide operations, mentor team members, monitor patient outcomes, and maintain the standards of care that define Hartford HealthCare at Home. This is a Monday through Friday position, 8:00 AM to 4:30 PM, based at our Hampden office.

What You'll Do

• Supervise and coordinate home care aide staff, including scheduling, training, and performance evaluation
• Assess patient needs and develop individualized care plans aligned with clinical best practices
• Ensure compliance with state regulations, licensing requirements, and Hartford HealthCare protocols
• Provide clinical oversight during home visits and monitor aide adherence to care standards
• Communicate with patients, families, physicians, and interdisciplinary care teams
• Document patient care, clinical findings, and supervisory activities in our electronic health record
• Support quality improvement initiatives and maintain patient safety standards

What We're Looking For

• Current, active LPN or LVN licensure in Massachusetts
• At least 2 years of clinical nursing experience (home care, long-term care, or hospital setting preferred)
• Demonstrated supervisory or leadership experience, or proven readiness to step into a leadership role
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to manage multiple priorities and work independently in a flexible environment
• Valid driver's license and reliable transportation
