About the Role

As a Hospice RN visiting nurse, you'll provide direct patient care in home settings, bringing expert nursing skills, clinical judgment, and empathy to individuals and families navigating end-of-life transitions. You'll work as an integral part of an interdisciplinary hospice team, combining clinical excellence with the flexibility and autonomy that home-based nursing offers.

What You'll Do

• Assess and monitor patient conditions, managing pain, symptoms, and comfort needs with evidence-based interventions
• Provide skilled nursing care including wound management, medication administration, and patient education
• Communicate compassionately with patients and families, addressing their physical, emotional, and spiritual needs
• Coordinate care with physicians, social workers, chaplains, and other hospice team members
• Maintain detailed clinical documentation and patient records
• Support families through the dying process and early bereavement when appropriate

What We're Looking For

• Current, unrestricted Registered Nurse (RN) license in Connecticut
• Strong clinical foundation with ability to work independently in home environments
• Compassionate approach to patient and family care
• Excellent communication and interpersonal skills
• Ability to manage multiple visits and prioritize clinical needs effectively
• Commitment to providing dignified, patient-centered care
