Manchester Memorial Hospital in Simsbury is hiring a Wound Care Nurse Practitioner (APRN) or Physician Assistant for a full-time inpatient position. As a wound care specialist, you'll provide expert clinical assessment and management for patients with complex wounds, ostomies, and continence challenges while collaborating with interdisciplinary care teams to drive optimal patient outcomes.

About the Role

Located at 71 Haynes Street in Simsbury, this wound care position centers on delivering specialized consultation across our inpatient units. You'll be responsible for comprehensive wound and ostomy evaluations, recommending evidence-based interventions, and ensuring that complex care needs are met through meticulous clinical attention and best-practice protocols.

What You'll Do

• Assess and evaluate wounds, ostomies, and continence conditions; recommend and implement clinical interventions
• Provide expert consultation to nursing staff and physicians on wound care protocols and management strategies
• Develop individualized patient care plans in collaboration with interdisciplinary teams
• Monitor treatment responses, adjust care plans as needed, and educate patients and families on wound prevention and self-care
• Maintain thorough clinical documentation supporting quality metrics and regulatory compliance standards

What We're Looking For

• Active Connecticut APRN or Physician Assistant license with demonstrated clinical experience in wound care, ostomy management, or continence nursing
• Wound, Ostomy, and Continence Nursing (WOCN) certification or equivalent specialized training preferred
• Strong clinical judgment and ability to work both independently and collaboratively
• Knowledge of evidence-based wound care protocols and standards of practice
• Excellent communication and leadership skills; ability to mentor and guide care teams
