About This Opportunity

We are representing a confidential healthcare organization transforming kidney care delivery through value-based, patient-centered approaches. Our client is building innovative care models that improve outcomes for patients living with chronic kidney disease, and we're seeking a Remote Care Coordinator (LPN) to join their connected care program.

This fully remote role requires availability during Pacific Time business hours (8:30am – 5pm PT) and offers the chance to make a meaningful impact in nephrology care coordination.

Responsibilities

  • Perform monthly clinical check-ins with assigned patient panels through SMS and phone outreach
  • Build ongoing relationships with patients and caregivers through regular engagement
  • Conduct structured symptom assessments including pain, fatigue, neuropathy, shortness of breath, and nausea evaluation
  • Review patient chart data including diagnoses, kidney disease history, medications, labs, and imaging before patient contact
  • Evaluate changes in patient condition using licensed clinical judgment and identify red flags requiring escalation
  • Assess medication adherence, side effects, and patient understanding of treatment regimens
  • Escalate urgent clinical issues to providers based on defined triage pathways
  • Collaborate with provider teams on symptom changes, new concerns, and medication-related issues
  • Reinforce care plans developed by providers and provide clinical education within LPN scope
  • Coordinate care activities including scheduling follow-ups, arranging labs, and reducing missed appointments
  • Document all patient interactions in both EHR and care management platforms with accuracy
  • Track qualifying time accurately for billing compliance and regulatory requirements
  • Maintain performance metrics including ≥80% monthly engagement and ≥90% billable conversion
  • Participate in peer reviews, quality audits, coaching sessions, and continuous education

What We're Looking For

Required Qualifications:

  • Active LPN or Licensed Vocational Nurse (LVN) license
  • Bilingual fluency in Spanish and English
  • Minimum 1–2 years clinical experience in ambulatory care, population health, care coordination, or case management
  • Strong clinical assessment, communication, and documentation skills
  • Ability to work effectively in two systems (EHR and care management platform) simultaneously
  • Capability to manage structured, metric-driven workflows reliably
  • Intermediate proficiency with MS Office Suite including Outlook and Teams
  • Home internet with minimum 25 Mbps download and 10 Mbps upload speeds (cable, fiber, or DSL hardwired connection recommended)
  • Dedicated workspace free from ongoing interruptions to protect patient PHI and maintain HIPAA compliance

Preferred Qualifications:

  • Experience in chronic disease management programs
  • Prior work in virtual care, remote nursing, or telehealth settings
  • Familiarity with chronic care management (CCM) regulatory requirements
  • Strong patient education and motivational interviewing skills

Pay

Our client offers a competitive compensation package for this role, with exact pay determined by experience, education, and role-specific factors. This position is also eligible for quarterly bonuses.
