Madison, OH, Provider Liaison- Medical Assistant Temporary

Theoria Medical is hiring a Medical Assistant for a temporary, full-time position in Madison, OH. In this role, you'll support residents, families, and clinical providers across skilled nursing facilities as a vital member of our post-acute care team.

What This Role Involves

As a Medical Assistant for Theoria Medical, you'll coordinate care and serve as the bridge between residents, families, nursing staff, and providers in the post-acute care environment. You'll facilitate clinical visits—both in-person and via telemedicine—manage care transitions, ensure accurate health information flows through our electronic health record system, and reinforce medical guidance to help residents achieve better health outcomes in our value-based care model.

Day-to-Day Responsibilities

• Coordinate and support telemedicine sessions along with scheduling routine and follow-up provider appointments for residents
• Prepare residents for provider visits, assist with positioning during examinations, and document visit details in the EHR
• Manage care transitions by coordinating referrals, scheduling follow-up appointments, and processing prior authorization requests
• Educate residents on medication management, nutrition, fall prevention, and wellness best practices
• Maintain organized, accurate documentation in the electronic medical record system to support quality and regulatory compliance
• Build relationships with facility administrators, case management teams, and attending physicians
• Participate in interdisciplinary care meetings and contribute clinical insights on patient progress
• Travel to skilled nursing facilities and hospitals as needed to coordinate care (up to 30 miles one way)

Skills & Qualifications

• Completed accredited Medical Assistant program
• Certified Medical Assistant (CMA) certification preferred
• Clinical or care-coordination experience as a Medical Assistant; health plan or hospice liaison experience valued
• Proficiency with electronic health records and Microsoft Excel
• Knowledge of geriatric medicine and terminology
• Strong written and verbal communication skills
• Excellent interpersonal abilities and collaborative mindset
• Ability to lift 20 lbs independently and stand for extended periods
• Detail-oriented, problem-solving approach with a commitment to quality patient care
