Gainesville, FL, Provider Liaison- Medical Assistant Temporary

Theoria Medical, a physician-led post-acute care organization in Gainesville, FL, is hiring a full-time Medical Assistant for a temporary position as a Provider Liaison—connecting residents, healthcare providers, clinical staff, and families within our skilled nursing facility network. This 90-day assignment (with possible extension) features 6-hour facility shifts generally starting between 7 and 10 a.m., with no on-call or overnight duties.

Role Overview

In this Provider Liaison Medical Assistant role based in Gainesville, FL, you'll serve as the vital hub coordinating patient care across our value-based care model. You'll manage provider interactions through telemedicine and in-person visits, oversee care coordination across the post-acute continuum, educate residents on health maintenance, and ensure families remain engaged in treatment decisions. Your work directly impacts patient outcomes and facility operations.

Day-to-Day Responsibilities

• Coordinate and facilitate telemedicine visits, acute appointments, follow-up consultations, and routine provider encounters
• Prepare residents for provider meetings, assist with positioning, compile relevant medical information, and complete clinical notes
• Record and update patient medical histories in electronic health records to inform comprehensive care planning
• Manage care transitions across hospitals and skilled nursing settings, including processing referrals and scheduling follow-up appointments
• Complete prior authorization requests and guide residents through ACO Voluntary Alignment enrollment
• Teach residents and families about nutrition, accident prevention, medication management, and wellness practices
• Escalate clinical questions to nursing leadership or attending physicians as appropriate
• Produce thorough, compliant EHR documentation supporting quality improvement and regulatory adherence
• Participate in interdisciplinary team conferences to deliver patient status updates and assist care planning
• Build collaborative relationships with facility leadership, case management, and social services departments
• Conduct occasional home visits to support community-based continuity of care

What You Bring

• Completion of an accredited Medical Assistant training program; CMA certification is a plus
• Clinical or care-coordination experience as a Medical Assistant; prior work in Health Plans or Hospice settings is valued
• Demonstrated competence charting in electronic medical record platforms
• Exceptional interpersonal and communication skills—both written and verbal
• Strong analytical ability, self-directed problem-solving, and a commitment to measurable quality results
• Solid grasp of aging-related medical conditions and clinical terminology
• Willingness to travel locally within a 30-mile radius; capable of lifting 20 lbs independently and assisting colleagues with transfers; stamina to work extended standing periods
• Polished professional presence, collaborative attitude, and creative problem-solving orientation
