About the Role

You'll lead clinical care at skilled nursing facilities across Troy, working 1–2 days per week as attending physician for assigned patient populations. Collaborate with nurse practitioners and physician assistants to coordinate care, drive quality initiatives in transitional care and chronic disease management, and leverage remote patient monitoring. The position emphasizes clinical leadership, evidence-based practice, and continuous professional growth.

Day-to-Day Responsibilities

• Serve as attending physician and medical director for skilled nursing facility patient populations
• Work with advanced practice clinicians to deliver comprehensive daily care and ensure clinical supervision
• Diagnose, treat, and manage acute and chronic medical conditions with a patient-centered approach
• Lead quality improvement initiatives including chronic care management, transitional care programs, and remote patient monitoring
• Document efficiently using an EHR platform with voice dictation and access to clinical resources

Qualifications

• MD or DO degree with an active U.S. medical license
• Board certification or board eligibility in a relevant specialty: Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine
• Demonstrated commitment to evidence-based post-acute care
• Strong collaboration and communication skills for working with interdisciplinary teams
