Role Overview

Theoria Medical brings physician-led post-acute care directly to patients in skilled nursing facilities rather than requiring them to travel to providers. As a Provider Liaison Medical Assistant, you'll serve as the essential link between patients, their families, nursing teams, and our clinical providers, supporting evidence-based transitions of care and helping residents achieve their health goals within a value-driven care model. Your work directly impacts patient outcomes and family satisfaction by ensuring smooth coordination and clear communication across the care team.

Core Duties

• Coordinate in-person and remote provider visits, scheduling appointments and preparing residents for care encounters
• Maintain and update patient records in the electronic health record system to support accurate care planning and clinical documentation
• Manage care transitions across post-acute settings, organizing referrals, follow-up arrangements, and authorization requests
• Educate residents and families on clinical topics including medication adherence, nutrition, fall safety, and preventive wellness practices
• Support discharge planning and coordinate with facility teams, case management, and social work departments to ensure continuity of care
• Visit skilled nursing facilities and hospitals regularly to monitor active patients and maintain strong working relationships with facility leadership and departments
• Route clinical questions and concerns to appropriate clinical or licensed staff members
• Attend interdisciplinary team meetings and provide patient-specific insights to support care decisions
• Conduct occasional home visits as needed by the care coordination team

What We're Looking For

• Completion of an accredited medical assistant training program; current Certified Medical Assistant (CMA) credential preferred
• Prior experience as a medical assistant in a clinical, health plan, hospice, or managed care setting preferred
• Proficiency charting in electronic medical record systems
• Excellent interpersonal and communication skills, both verbal and written
• Strong attention to detail and ability to manage multiple patients and tasks independently
• Familiarity with geriatric patient care and medical terminology
• Proficiency with advanced computer applications, spreadsheets, and email systems
• Professional demeanor and commitment to ethical patient care
• Ability to lift up to 20 pounds independently and assist with patient transfers as part of a team
• Capability to stand for extended periods and travel to patient locations within a 30-mile radius of Gainesville
• Availability for 6-hour facility shifts as part of this full-time position; no on-call or overnight work required
• Flexibility to commit to a 90-day assignment with potential for extension
