Clearwater, FL, Provider Liaison- Medical Assistant Temporary

Join Theoria Medical's care team as a Provider Liaison–Medical Assistant in Clearwater, FL, where you'll support skilled nursing residents by coordinating clinical services, managing provider interactions, and championing patient wellbeing in a fast-paced, outcomes-focused environment. This full-time temporary position operates on 6-hour facility shifts with rounds typically starting between 7 and 10 a.m., no on-call duties, and a 90-day assignment period.

What This Role Involves

This position centers on bridging the gap between residents, clinical providers, nursing teams, and families. You'll manage the day-to-day logistics of resident care—from scheduling telehealth sessions to preparing patients for in-person visits—and ensure smooth handoffs across the continuum of post-acute services. Your work directly impacts resident health outcomes and satisfaction in skilled nursing settings across the region.

Core Duties

• Schedule and facilitate telemedicine visits and routine provider appointments; prepare residents and complete visit support documentation
• Update patient records in the electronic health record system with medical histories and care plan information
• Manage care transitions, including coordinating referrals, follow-up scheduling, and processing prior authorization requests
• Educate residents on medication adherence, nutrition, fall safety, and wellness topics using provider-approved materials
• Route clinical concerns to nursing staff or providers and flag urgent matters for immediate attention
• Maintain accurate, compliant documentation supporting quality and regulatory standards
• Build and maintain relationships with facility leadership, admissions staff, social work, and case management teams
• Represent Theoria Medical during facility visits, attending interdisciplinary meetings and serving as a resource for patients and families
• Support discharge planning and assist with logistics such as acquiring clinical records and coordinating ancillary services
• Travel to patient locations (hospitals, skilled nursing facilities, homes) within a 30-mile radius as needed

What You Bring

• Completion of an accredited Medical Assistant program; Certified Medical Assistant (CMA) credential preferred
• Demonstrated experience as a Medical Assistant in clinical or care-coordination roles, or prior experience in health plan liaison, hospice, or managed care settings
• Proficiency charting in electronic medical records systems
• Strong interpersonal skills and ability to communicate clearly with diverse audiences—residents, families, clinical staff, and administrators
• Detail-oriented approach, self-directed problem-solving ability, and commitment to ethical patient care
• Working knowledge of geriatric medicine and clinical terminology
• Intermediate to advanced computer skills, including spreadsheet functions, email, and productivity software
• Physical capability to lift 20 pounds independently, assist with resident transfers, stand for extended periods, travel to multiple sites, and perform detailed clinical work requiring visual acuity and manual dexterity
• Collaborative, flexible mindset with a focus on continuous improvement and quality-driven outcomes
