Lima, OH, Provider Liaison- Medical Assistant Temporary

Theoria Medical seeks a Medical Assistant for a temporary full-time role in Lima, Ohio, to support integrated patient care within skilled nursing facilities. In this position, you'll facilitate clinical visits, manage care coordination, educate residents, and ensure seamless communication among healthcare teams—directly influencing patient outcomes through our value-based care model.

The Opportunity

Theoria Medical delivers physician-led clinical services directly into skilled nursing facilities, reshaping post-acute care through integrated technology and evidence-based health models. We're building a new approach to senior care—one where clinical teams embed within facilities rather than waiting for patients to travel elsewhere. As a Medical Assistant Provider Liaison in Lima, Ohio, you'll be central to this model: orchestrating clinical visits, managing care continuity, educating residents, and serving as the consistent presence that ties everything together. You'll work 6-hour facility shifts, typically starting between 7 and 10 a.m., with no on-call or overnight responsibilities.

Responsibilities

• Coordinate telemedicine sessions and in-person provider appointments, including pre-visit preparation, documentation, and post-visit follow-up scheduling
• Update and maintain electronic health records with current resident information to support clinical planning and care continuity
• Facilitate care transitions across settings (hospitals, SNFs, home environments), managing referrals, authorizations, and enrollment procedures
• Educate residents and families on evidence-based practices including nutrition, fall prevention, medication management, and preventive wellness
• Attend interdisciplinary team meetings and communicate clinical updates between facility and external care providers
• Organize family care conferences and support discharge planning to align patient preferences with clinical recommendations
• Build professional working relationships with facility administrators, case managers, social workers, and clinical staff
• Access, review, and transfer medical records to ensure care continuity and regulatory compliance
• Represent our organization within facilities, serving as patients' trusted point of contact when providers are not physically present
• Participate in home visits when needed to coordinate community-based care elements
• Perform detailed documentation to meet regulatory standards and quality reporting requirements

Who We're Looking For

• Accredited Medical Assistant graduate; Certified Medical Assistant (CMA) credential preferred
• Previous Medical Assistant experience in clinical care coordination, health plan operations, or hospice settings strongly valued
• Comfortable and proficient with electronic medical records systems and intermediate computer skills (spreadsheet functions, email platforms, online collaboration tools)
• Excellent written and verbal communication abilities, with a professional, empathetic demeanor
• Organized, detail-focused approach to managing competing priorities and meeting deadlines
• Working knowledge of geriatric conditions, medical terminology, and age-related care considerations
• Proactive problem-solver who takes ownership of outcomes and works well independently and as part of a team
• Dedicated to ethical, respectful patient care and genuine collaboration with multidisciplinary clinical staff
• Physically capable of local travel (up to 30 miles one way), standing for extended periods, and lifting up to 20 pounds independently; ability to assist with resident mobility as part of a care team
