Join PATH as a Metro Line Substance Use Specialist and create real pathways out of homelessness for people throughout Los Angeles County. This full-time position on our Metro Rail Outreach Team combines street engagement, substance use assessment, and case management—delivering hope and housing opportunities where they're needed most.

About the Role

You'll work Monday through Friday from 9:30am to 6:00pm as part of PATH's Metro Rail Outreach Team, providing direct street outreach and evidence-based case management services to people experiencing homelessness across the MTA transit system. Your mission: build trust, identify needs, and connect clients to the housing, treatment, and supportive services that stabilize their lives. You'll work both independently and collaboratively to remove barriers and unlock pathways to long-term housing stability.

What You'll Do

  • Conduct street-level engagement at Metro stations, bus stops, and aboard trains to develop relationships and build trust with clients experiencing homelessness.
  • Administer standardized vulnerability assessments (VI-SPDAT) to identify client priorities and match them with appropriate interventions and service referrals.
  • Evaluate substance use challenges and connect clients to treatment programs, recovery resources, and peer support services.
  • Deliver street-based case management support, including benefits navigation (SSI, TANF, veteran pensions), identification assistance, employment support, and housing placement.
  • Connect clients to medical care, mental health services, and other resources that improve stability and wellbeing.
  • Conduct regular check-ins with established clients to track progress, celebrate milestones, and sustain momentum toward housing and recovery goals.
  • Respond promptly to outreach requests and coordinate transitions from street situations to interim and permanent housing.
  • Collaborate with substance use treatment providers, mental health agencies, and social service organizations to ensure coordinated, seamless care.
  • Work with law enforcement and housing providers to facilitate smooth transitions and continuity of services.
  • Identify and respond to trends and emerging needs within the homeless population served.
  • Maintain complete, accurate client documentation in HMIS and all required tracking systems.
  • Participate in team meetings, professional trainings, and ongoing skill development.

What We're Looking For

Required Qualifications:

  • Minimum 2 years of direct experience in substance use services.
  • Valid driver's license with reliable personal transportation; proof of current insurance required (driving is essential to this role).
  • Ability to qualify for PATH's insurance coverage for work vehicle use.
  • Current CPR and First Aid certification, or willingness to obtain it as a condition of employment.
  • U.S. work authorization (PATH cannot sponsor visa applications).
  • Pass background screening, tuberculosis screening, and drug screening as conditions of hire.
