About This Opportunity

We are representing a mission-driven healthcare organization that partners with community health centers nationwide to transform care delivery and reduce health disparities. Our client is seeking a Clinical Services Coordinator to join their remote clinical operations team.

In this role, you'll serve as a vital connection between patients, providers, and care teams—conducting outreach for preventive care programs, coordinating transitions of care, supporting chronic disease management, and helping close gaps in care. You'll work with population health technology, AI-assisted tools, and EMR platforms to identify patients who need wellness visits, follow-up appointments, and preventive screenings.

Responsibilities

  • Conduct proactive patient outreach for Annual Wellness Visits, Transitions of Care follow-up, preventive screenings, and chronic disease management programs
  • Schedule and coordinate appointments across primary care, specialty care, behavioral health, and community organizations while addressing barriers to access
  • Utilize EMR systems, population health registries, and AI-enabled tools to identify patients due for services and quality interventions
  • Support post-discharge coordination including appointment scheduling, medication review facilitation, and care plan reinforcement
  • Provide patient education on self-management, navigate healthcare services, insurance resources, transportation, and community supports
  • Collaborate with interdisciplinary care teams to improve patient engagement, reduce care gaps, and support value-based care goals
  • Conduct standardized screenings including health risk assessments and social determinants of health evaluations as appropriate
  • Maintain accurate, timely documentation in EMR and care management platforms in compliance with privacy standards and organizational policies
  • Participate in quality improvement initiatives, workflow optimization, and implementation of new clinical programs
  • Escalate clinical, behavioral health, or social concerns to licensed clinicians or appropriate team members

What We're Looking For

  • 3+ years of experience in care coordination, care/case management, or patient outreach with community resource navigation
  • Bilingual in Spanish with fluency in medical terminology (required)
  • Experience working in one or more of these settings: safety-net hospitals, Federally Qualified Health Centers (FQHCs), academic medical centers, ambulatory care, physician practices, community mental health centers, substance use treatment, or outpatient mental health services
  • Current state licensure as RN, LPN, LSW, or LCSW is a plus (additional state licensure may be required where patient care is provided)
  • Demonstrated experience engaging patients with chronic illnesses, behavioral health needs, and social factors affecting health outcomes
  • Knowledge of Medicaid and Medicare populations and value-based care models desirable
  • Comfort with technology platforms including EMR systems, population health registries, and AI-assisted engagement tools
  • Strong communication and relationship-building skills with patients, caregivers, providers, and community partners
