Speech Language Pathologist

Phoenix Home Care and Hospice is seeking a part-time Speech Language Pathologist to join our clinical team in Lebanon, Missouri. Role Overview

As a Speech Language Pathologist with Phoenix Home Care and Hospice, you'll provide individualized assessment and treatment services to patients in their homes and care settings throughout Lebanon and surrounding communities. This position combines clinical expertise with the flexibility of part-time scheduling, allowing you to make a meaningful impact while managing your own professional commitments. You'll work collaboratively with interdisciplinary teams to support patients facing speech, language, voice, swallowing, and cognitive challenges related to acute illness, chronic conditions, and end-of-life care.

Your Responsibilities

• Conduct comprehensive speech, language, voice, and swallowing evaluations for patients in home and hospice settings
• Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings
• Provide direct patient care and therapy sessions, including swallowing assessments and dysphagia management
• Document clinical findings, treatment progress, and outcomes in accordance with regulatory standards
• Collaborate with physicians, nurses, social workers, and other team members to ensure coordinated, person-centered care
• Educate patients, families, and caregivers on communication strategies and safety precautions
• Maintain current knowledge of evidence-based practices in speech-language pathology

Qualifications

• Master's degree or doctoral degree in Speech-Language Pathology from an accredited program
• Current Clinical Competence Certificate (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA)
• Valid state licensure to practice as a Speech Language Pathologist in Missouri
• Strong clinical experience, preferably in home health, hospice, or acute care settings
• Excellent communication and documentation skills
• Compassionate approach to patient and family interactions
• Reliable transportation and valid driver's license
