Caseworker

Cencora is seeking a dedicated Caseworker to join our team in Canada. This full-time position offers the opportunity to make a real difference by supporting individuals and helping them access the resources they need.

Your Responsibilities

• Conduct comprehensive assessments to understand each client's needs, circumstances, and goals
• Develop and implement individualized case plans in collaboration with clients and other service providers
• Maintain detailed, accurate case records and documentation to track progress and outcomes
• Connect clients with appropriate community resources, social services, and support programs
• Conduct follow-up visits and consultations to monitor client progress and adjust support as needed
• Advocate on behalf of clients to help them access benefits and services they are entitled to
• Communicate regularly with clients, supervisors, and partner organizations to ensure coordinated care

Skills & Qualifications

• High school diploma or equivalent; relevant post-secondary education or certification in social work or a related field is an asset
• Excellent verbal and written communication skills
• Strong organizational and time management abilities
• Demonstrated compassion, empathy, and cultural awareness when working with diverse populations
• Ability to maintain confidentiality and handle sensitive information responsibly
• Comfort with computer systems and willingness to use case management software
• Problem-solving mindset and ability to work both independently and collaboratively
