Join Hartford HealthCare at Home as a full-time Licensed Practical Nurse (LPN) delivering compassionate visiting nurse services to patients throughout the Connecticut shoreline, serving communities including Stonington, Groton, Waterford, East Lyme, Old Lyme, Old Saybrook, Mystic, Noank, and North Stonington in the Tolland County area.

About the Role

As a visiting nurse at Hartford HealthCare at Home, you'll provide hands-on patient care directly in home settings across the shoreline region. This is meaningful clinical work where your skills and compassion create measurable improvements in patient health and quality of life. You'll function as a full-time member of our home health team, delivering skilled nursing care to patients who depend on in-home services for recovery, maintenance, and support.

What You'll Do

• Provide skilled nursing care to patients in their homes, focusing on recovery, health maintenance, and comfort
• Assess patient conditions, monitor vital signs, and communicate clinical changes to the care team
• Perform clinical procedures and wound care within your LPN scope of practice and facility protocols
• Collaborate with physicians, therapists, and care coordinators to develop and implement patient care plans
• Maintain detailed patient records and documentation to ensure continuity of care
• Educate patients and families about health management, medication compliance, and self-care strategies

What We're Looking For

• Current Connecticut LPN license in active good standing
• Experience in home health nursing, visiting nurse services, or similar home-based clinical setting (preferred)
• Valid driver's license and reliable transportation for traveling to patient homes across the service region
• Strong assessment and clinical skills with ability to work independently while maintaining excellent team communication
• Genuine compassion for patients and commitment to respectful, dignity-centered care
• Professional adaptability and sound judgment in diverse home environments
