About the Role

The Hospital of Central Connecticut is seeking a clinically experienced NP or PA to join its primary care team. You'll work in a hospital-based primary care environment where you'll manage patient panels, deliver preventive and acute care, and collaborate with physicians and interdisciplinary teams. This full-time position offers the clinical autonomy and professional support you need to practice at the top of your license while contributing to a strong community health mission.

What You'll Do

• Evaluate, diagnose, and treat a broad spectrum of primary care conditions across adult and geriatric patient populations
• Develop and execute comprehensive treatment plans with an emphasis on preventive health and chronic disease management
• Order and interpret diagnostic studies, manage prescriptions, and coordinate specialist referrals
• Build and maintain strong patient relationships while ensuring evidence-based clinical standards and quality outcomes
• Collaborate with hospital physicians, nursing staff, and care teams on clinical protocols and patient care initiatives
• Participate in quality improvement projects and stay current with clinical advances in primary care medicine

What We're Looking For

• Current, unrestricted Nurse Practitioner or Physician Assistant license in Connecticut
• Demonstrated clinical experience in primary care or internal medicine
• Commitment to evidence-based practice and patient-centered care delivery
• Strong clinical judgment, communication skills, and ability to work collaboratively in a team environment
