About This Opportunity

We are representing a confidential healthcare organization in their search for a Transition Coordinator to join their Medicaid-focused care management team. This remote role is based in Indiana and requires residence in Clark, Floyd, Harrison, or Scott County.

In this position, you'll serve as a vital advocate for members navigating transitions between care settings—from hospitals to home, from institutional to community-based care. Your work will directly impact members' ability to achieve wellness in the least restrictive setting appropriate for their needs.

Responsibilities

  • Coordinate ongoing member transitions in and out of state Medicaid programs and among various care settings
  • Conduct telephonic and in-person meetings within your assigned region with members, hospital discharge planners, family members, primary care physicians, and other healthcare professionals
  • Assess and evaluate member requirements to establish individualized care plans and coordinate appropriate services
  • Ensure transfer and receipt of prior authorization decisions, utilization management data, and clinical information including care management notes
  • Facilitate transitions from custodial settings to home and community-based settings, working to prevent institutional placements when possible
  • Guide members and families toward appropriate resources and facilitate interactions with payer sources, providers, and interdisciplinary teams
  • Collaborate with member advocate coordinators and other departments to ensure continuity and coordination of care
  • Maintain documentation in electronic case management systems

What We're Looking For

Required Qualifications:

  • Bachelor's degree in a health and human services field
  • 2+ years of relevant experience in care coordination or case management
  • Valid driver's license and personal vehicle liability insurance meeting state requirements (minimum $25,000 bodily injury per person/$25,000 per event/$10,000 property damage or higher)
  • Must reside in Clark, Floyd, Harrison, or Scott County, Indiana
  • Dedicated home office space with a locked door to ensure privacy and HIPAA compliance
  • Self-provided internet service (minimum 25 Mbps download/10 Mbps upload; wired cable or DSL recommended)
  • Willingness to undergo TB screening as this is a patient-facing role
  • Passion for improving consumer healthcare experiences

Preferred Qualifications:

  • Master's degree in related field
  • Applicable state licensure
  • Case Management Certification (CCM)
  • Experience with Interqual or Millman utilization review criteria
  • Prior experience working with Medicare and Medicaid populations
  • Experience with electronic case note documentation across multiple computer systems
  • Background in health promotion, coaching, and wellness
  • Knowledge of community health, social service agencies, and local resources
