Physical Therapist- Greater Livingston, NJ

Join Fox Rehabilitation as a Physical Therapist serving the Lake Swannanoa area of Passaic County and Greater Livingston, NJ. This full-time position delivers evidence-based physical therapy to older adults in their homes and communities across the region.

What This Role Involves

You'll work with an established practice specializing in geriatric care delivered in patients' home environments across multiple New Jersey locations. As a Physical Therapist, you'll conduct functional assessments, design individualized treatment plans, and guide patients through therapeutic interventions within real-world community settings. This role emphasizes clinical expertise, patient-centered care, and adaptability to diverse home-based environments where older adults live and age in place.

Responsibilities

• Evaluate patients' physical function, balance, strength, and mobility using evidence-based clinical assessment
• Develop and implement personalized physical therapy treatment plans based on individual patient goals and home environment
• Deliver skilled therapeutic interventions including manual therapy, therapeutic exercise, and functional training
• Monitor patient progress, adjust treatment protocols, and communicate outcomes with patients and families
• Maintain accurate clinical documentation and ensure compliance with regulatory and quality standards
• Coordinate care with interdisciplinary teams to optimize patient outcomes

Requirements

• Current New Jersey Physical Therapist license
• Bachelor's degree or higher in Physical Therapy from an accredited institution
• Professional experience or demonstrated commitment to geriatric patient care
• Ability to travel to multiple patient locations throughout Livingston, Roseland, West Orange, Maplewood, and surrounding communities
• Strong interpersonal and communication skills
• Dedication to patient-centered, evidence-based clinical practice
