Licensed Clinical Social Worker (Texas)

About This Opportunity

Our talent team is representing a confidential value-based palliative care organization seeking a Licensed Clinical Social Worker to join their interdisciplinary care team. This is a remote position serving patients across Texas—you must hold an active Texas LCSW license and reside in the state.

In this role, you'll provide comprehensive psychosocial and emotional support to individuals with serious illness and their families. You'll conduct assessments, facilitate advance care planning conversations, deliver clinical counseling, and collaborate with physicians, nurses, and care managers to improve quality of life and reduce avoidable hospitalizations.

Responsibilities

• Conduct comprehensive psychosocial assessments evaluating mental health needs, caregiver burden, social determinants of health, and risk factors for patients with serious illness
• Facilitate serious illness conversations and advance care planning with patients and families, ensuring documentation aligns with patient goals and values
• Provide clinical recommendations and escalate concerns to physicians and advanced practice providers regarding safety, mental health, caregiver stress, or complex psychosocial needs
• Develop and document clinically appropriate interventions within individualized care plans
• Participate in interdisciplinary rounds and case conferences, contributing clinical insights to support coordinated, goal-aligned care
• Deliver short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers coping with illness progression and end-of-life considerations
• Utilize electronic health records and clinical documentation systems to support care coordination and quality measurement
• Establish therapeutic, trust-based relationships grounded in clinical listening, empathy, and culturally responsive care
• Support the care team through debriefings following emotionally charged cases and facilitate periodic memorial rounds

What We're Looking For

• Master's in Social Work (MSW) from an accredited program
• Active, unrestricted Licensed Clinical Social Worker (LCSW) license in Texas (required)
• Clinical experience in palliative care, including advance care planning and serious illness conversations (required)
• Experience working within interdisciplinary care models alongside physicians, APPs, nurses, and care managers
• Demonstrated ability to exercise independent clinical judgment and appropriately escalate urgent or emergent situations
• Strong verbal and written communication skills with clear clinical documentation abilities
• Excellent organizational and time-management skills to manage multiple priorities effectively
• Proficiency with electronic health records and digital clinical tools
• Ability to thrive in a fast-paced, evolving environment with flexibility and resilience
• Commitment to compassionate, patient-centered care aligned with value-based care principles
