Home Health Physical Therapist PT Full Time

Aveanna Healthcare is hiring a Physical Therapist to deliver therapeutic care in patients' homes throughout Sioux Falls, Minnehaha County. In this Full Time position, you'll help patients recover from illness and injury by providing expert physical therapy services in their own living environments.

About the Role

Our home health team at Aveanna Healthcare provides essential rehabilitative services to patients recovering from acute illness, surgery, and injury. As a Physical Therapist with our Sioux Falls practice, you'll work directly with patients to restore function, reduce pain, and improve their quality of life during the critical recovery period.

What You'll Do

• Evaluate patients in their home environments and develop personalized physical therapy treatment plans
• Deliver hands-on therapeutic interventions, including manual therapy and exercise training
• Monitor patient progress, adjust treatments, and document clinical outcomes
• Teach patients and caregivers therapeutic exercises and safety strategies for home independence
• Coordinate care with nursing, occupational therapy, and social services on the interdisciplinary team
• Complete comprehensive clinical documentation and maintain patient medical records
• Determine discharge timing and recommend community resources for ongoing wellness

What We're Looking For

• Current Physical Therapist license to practice in South Dakota
• Master's degree or Doctor of Physical Therapy (DPT) from an accredited institution
• Valid driver's license and reliable transportation to visit patient homes in the Sioux Falls area
• Excellent communication and interpersonal abilities
• Experience or willingness to learn home health practices and patient-centered care approaches
• Professional commitment to helping patients achieve functional independence
