Home Health Licensed Practical Nurse LPN Part Time

Join Aveanna Healthcare as a Licensed Practical Nurse (LPN) providing skilled nursing care to home health patients in the Capshaw area of Limestone County. We're seeking a compassionate, detail-oriented nurse to deliver quality patient-centered care across our Madison County service territory on a flexible Monday–Friday visit schedule.

About the Role

As part of our home health team, you'll deliver evidence-based nursing services directly in patients' homes, building meaningful relationships while maintaining strict confidentiality and professional boundaries. You'll work collaboratively with physicians, care coordinators, and support staff to ensure each patient receives comprehensive, individualized care.

What You'll Do

• Provide skilled nursing interventions aligned with each patient's personalized care plan and physician orders
• Conduct patient assessments, monitor vital signs, and document clinical observations in real time
• Educate patients and caregivers on medication management, wound care, equipment use, and disease management
• Maintain detailed, accurate clinical records and communicate patient status updates to the care team
• Identify and report changes in patient condition or safety concerns promptly
• Uphold HIPAA compliance and maintain professional, respectful patient interactions

What We're Looking For

• Active, unrestricted Licensed Practical Nurse (LPN) license in Alabama
• Strong clinical nursing skills with home health or long-term care experience preferred
• Ability to work independently while collaborating effectively with an interdisciplinary team
• Excellent documentation and communication skills
• Valid driver's license and reliable transportation
• Compassion, reliability, and commitment to patient-centered care
