Home Health Registered Nurse RN Full Time

Aveanna Healthcare is seeking a dedicated Registered Nurse (RN) to join our Home Health team in Sioux Falls, Minnehaha County. In this full-time position, you'll provide skilled nursing care directly to patients in their homes, making a real difference in their health outcomes and quality of life.

Working a Monday through Friday schedule, you'll be a trusted clinical leader in home-based care—assessing patient needs, managing complex conditions, and supporting families through critical health transitions.

What You'll Do

• Assess patient health status and develop comprehensive individualized care plans
• Administer medications, manage wounds, and perform skilled nursing procedures
• Monitor for changes in patient condition and adjust care strategies accordingly
• Document clinical findings accurately and maintain detailed medical records
• Communicate with physicians and coordinate care with interdisciplinary teams
• Educate patients and caregivers about medications, disease management, and prevention
• Maintain safety standards and infection control in diverse home environments

What We're Looking For

• Active RN license in South Dakota or valid multi-state compact license
• Prior nursing experience, preferably in home health or community-based settings
• Strong clinical assessment, critical thinking, and decision-making skills
• Excellent interpersonal and patient education abilities
• Ability to work independently and manage your schedule effectively
• Commitment to patient-centered, compassionate care
• Valid driver's license and reliable transportation
