Good Samaritan is hiring a Registered Nurse Case Manager for our Home Health program in Northrop, Martin County. This is an opportunity to deliver patient-centered care management within a mission-driven organization serving seniors with dignity and compassion.
About the Role
As an RN Case Manager, you'll serve as the clinical hub connecting patients, families, and healthcare providers. You'll conduct comprehensive home assessments, develop individualized care plans, and ensure patients receive coordinated, high-quality support—all while advancing our commitment to excellent care delivered with compassion.
What You'll Do
- Perform in-home nursing assessments and evaluations to establish baseline health status and care needs
- Develop and update individualized care plans in collaboration with patients, families, and interdisciplinary care teams
- Coordinate services across providers, ensuring continuity and preventing gaps in care
- Educate patients and families about treatment, medications, lifestyle modifications, and available resources
- Monitor patient progress, adjust care plans as needed, and document all clinical findings thoroughly
- Serve as a liaison between patients and physicians, specialists, and social services
- Advocate for patients' needs and preferences throughout their care journey
What We're Looking For
- Active Minnesota RN licensure (or eligibility for licensure)
- Experience in home health nursing, case management, or related clinical settings
- Strong clinical assessment and critical thinking abilities
- Excellent communication and patient education skills
- Ability to work independently and manage time across multiple patient caseloads
- Compassion for seniors and commitment to person-centered care
- Valid driver's license and reliable transportation
