About the Role

This part-time RN Case Manager position offers day shifts in our home health program, where you'll coordinate comprehensive care for patients in their homes. Working 32 hours per week, you'll serve as the clinical leader connecting patients, families, physicians, and support services to ensure seamless, high-quality care delivery.

What You'll Do

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate care across multiple disciplines and healthcare providers
• Monitor patient progress, adjust care plans as needed, and maintain clinical documentation
• Communicate with physicians, therapists, social workers, and other care team members
• Educate patients and families about medications, treatments, and self-care management
• Review insurance authorizations and manage coverage issues
• Complete thorough medical documentation and maintain accurate patient records
• Assess patients' home environments for safety and accessibility needs

What We're Looking For

• RN licensure in Minnesota (current and active)
• Experience in home health nursing or case management preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage multiple patient relationships
• Commitment to patient-centered care in a faith-based environment
• Reliable transportation for home visits
